Community Healthcare Network Williamsburg

Community Healthcare Network (CHN) is a nonprofit organization that offers primary care, mental health services, and social services in Williamsburg, Brooklyn. These locations are designated by the Bureau of Primary Health Care as Federally Qualified Health Centers. CHN is a member of NewYork-Presbyterian Hospital. For more information, visit chn.nyc.org. Founded in 2001, Community Healthcare Network is a nonprofit group with one location. With 11 physicians and six locations, they serve the residents of Williamsburg, Brooklyn. Their mission is to provide quality health care to all, regardless of their income or background. The hospital is also accredited by the Joint Commission on Accreditation of Health Care Organizations. In addition to a patient-centered medical home, they provide preventive health care services.

In 2002, Sergio Matos, a CHW, and Elena Schwolsky, a public health nurse, founded the CHW Network. The goal was to provide training, professional development, and networking opportunities for their fellow CHWs. The United Hospital Fund funded focus groups with CHWs in Williamsburg, Brooklyn, and in other neighborhoods throughout New York. The findings were published in the Journal of the American Public Health Association and Progress in Community Health Partnerships.
