On January 1, 2012 the Community Clinic Association of San Bernardino County began working on a Specialty Care Initiative that has been funded by Kaiser Permanente Community Benefits Program.
The Community Clinic Association of San Bernardino County was awarded a 5 year grant by Kaiser Community Benefits Program to research and implement changes within San Bernardino County to improve access to specialty care for the safety net population. The San Bernardino County Specialty Care Initiative has three main goals that will be achieved over the next 5 years.
Goal 1: Develop a strategic planning process to improve the system of specialty care services in San Bernardino County.
Goal 2: Increase access to and utilization of specialty care services and communication between health care specialists and primary care providers.
Goal 3: Create and effective system for patient education and navigation related to specialty care.
In order to achieve these goals a coalition of members from throughout the county have begun coming together to look at the potential opportunities to help increase access and utilization of specialty care. Current coalition partners are:
- Arrowhead Regional Medical Center
- Inland Empire Health Plan
- San Bernardino County Medical Society
- Loma Linda University
- Western University
- University California Riverside School of Medicine
- San Bernardino County Department of Health
- Community Clinic Association of San Bernardino County clinic CEO’s
The Specialty Care Initiative will focus on improving access and utilization for the entire county of San Bernardino.
The goals of the Specialty Care Initiative will be researched, developed and implemented over the next 5 years.
